Please complete this intake questionnaire as part of your registration with 海角社区 Disability Services (NDS).
海角社区 (海角社区) is committed to providing reasonable accommodations to qualified students with disabilities to ensure equal access to educational opportunities. Accommodations are determined through an interactive process based on disability-related needs and supporting documentation.
I understand that completing this form is only the first step in the accommodation process. To be eligible for accommodations, I must provide documentation that verifies the functional impact of my disability(ies). Once registered with 海角社区 Disability Services, I must request accommodations each semester by communicating with the Director of Disability Services to develop and accommodation letter that will be provided to necessary college personnel.
I understand that I play an active role in the accommodation process. It is my responsibility to request accommodations, communicate with instructors regarding approved accommodations, and contact Disability Services if I have questions or concerns.
I understand that accommodations provide access to course content, instruction, and assessments and do not change essential learning outcomes or program requirements.

These accommodations are provided to assist students with disabilities in accessing education at 海角社区. Working in partnership with the student, the Disability Services Counselor will develop an individual plan for services that will include accommodations in testing and instruction. A student with disabilities must submit documentation to verify presence of a disability and request services each semester. Please note that program requirements will not be altered and standards will not be lowered.
